Kimberworth Park Social Prescribing Service is contracted to KPCP by Rotherham NHS Clinical Commissioning Group (managed via Voluntary Action Rotherham)

Our approach offers 1-1 support, mainly to older people who are experiencing long term physical and mental ill health conditions.  This enables those involved to explore the barriers created by their conditions and supports them in looking at ways where involvement and inclusion in community services, activity and life will have a positive impact on their overall health and wellbeing.

The measured impact outcomes, through consultation and evaluation, have indicated an increase in people’s self- confidence, an awareness of what is open and available to them, with a consequent effect on their ability to self- determine and remain independent.
