KPCP plays a part in the Rotherham-wide Social Prescribing pilot project. Through this service we work with people who have long-term health conditions and are referred to us by their GP surgery. Although most people referred to us are older, the service is for adults of all ages. We are working with these people to reduce social isolation by supporting them to attend various social activities of their choice, and enabling them to access free taster sessions without obligation.
We can link in to home visiting services for those who are unable or lack the confidence to go out initially. It is also part of our role to ensure that patients are aware of and can access statutory services, benefits advice and transport links. Overall, the project aims to have a positive impact upon the health of patients which may be reflected in hospital and GP surgery appointments.
The project is enhanced by the involvement of a small team of volunteers who help us to provide a Befriending service and social activities within the Kimberworth Park area, and we would welcome more volunteers to enable us to develop services.
